Boulder Physiolab utilizes an online scheduling & intake system, and fully contactless payment system via Square. No charges will be processed until your appointment begins. If you are a returning patient who has not received treatment since April 2020, you will need to enter the scheduling system as a new patient.

Her whole practice is designed based on 20+ years of experience and seeing what works, and what doesn’t in today’s healthcare realm. She truly cares about your experience in physical therapy and your ability to achieve your goals, and strives to ensure that each and every patient feels cared for and supported. She is a board certified orthopedic clinical specialist with a doctorate in physical therapy who has extensive knowledge and experience in all realms of Boulder PT, and is a former academic and researcher in the field. She is a certified yoga teacher as well.

She is dedicated to helping the community understand the tipping point between health and injury, and to promoting understanding of the biomechanics of movement patterns that allow the body to participate in a balanced life. She uses a blend of evidence-based medicine, manual therapy, functional training and exercise to help each individual she works with maintain a healthy and pain-free lifestyle.

What to Expect

If it’s your first time to physical therapy… welcome, and here are some helpful tips. If you are a returning patient, welcome back- and if you’re seeking help for an issue you’ve already been here for, let’s figure out together how to prevent a repeat in the future.

For your first visit, Dr. Haas will be completing a thorough evaluation of your movement patterns, strength & stability, mobility, and functional movements that you use in daily life that are related to the issue you are seeking help with. She will identify the source of your pain, compensation, or issue and apply these findings to create an individualized treatment plan that is best for you. She will discuss your goals and any concerns, and will clarify her advice and treatment plan with you to answer any outstanding questions that have not been covered during the evaluation process. Her goal with your first visit is to find the source of the issue and to start pointing you in the right direction, with advice, treatment, and any relevant exercises. All at our Boulder PT facility.

Clothing. Be sure to wear clothing that is comfortable for movement and exercise, and that allows easy access to the body part you are seeking help with. If you are undergoing a run analysis, be sure to your running shoes- old and new.

Intake Forms. Please be sure to complete the online intake form 24 hours in advance of your appointment, to allow time for review.

Medical records. Please email any records or reports that might be helpful or pertinent for your issue to Nicole@PTBoulder.com 24 hours in advance. Please include any operative reports, imaging reports (xrays, MRIs, CT scans), and any prescription from your referring provider (if applicable not required).

Insurance

Dr. Haas is dedicated to providing quality care as a physical therapist, and works hard to help each patient achieve their goals and potential. She strives to return her patients to a healthy and pain-free lifestyle through a caring, thought-filled, and evidence-based approach.

She does not contract with insurance companies in order to provide a full hour of individualized care, and to utilize those longer treatment times to achieve effective results and outcomes in fewer visits. Using this model of care, she is able to provide ongoing support and extended care between appointments via email and phone as needed. Her goal is to provide highly effective treatments with superior outcomes, and to do so in fewer sessions than the insurance-based style of PT.

Dr. Haas is able to accept payments through HSA cards (Health Savings Account) or your Flexible Spending Account (FSA). For patients wishing to use insurance who have “out of network benefits for physical therapy,” most PPOs will pay a portion of your treatment costs depending on your specific plan and chosen benefits. Dr. Haas is more than willing to help you navigate the process or answer any questions as well, and can provide a “superbill” for submission to your insurance company for reimbursement under “out of network” benefit coverage.
